Retrovesical Pelvic Hydatid Cysts A Case Series Review

Abstract

ABSTRACT 

Background: Hydatid disease is a zoonotic infection caused by the larval stage of a tapeworm particularly Echinococcus granulosus. It is considered an important public health problem in many areas of the world. It is one of serious endemic parasitic diseases in Iraq

Objective:  Review of the clinical findings of a series of pelvic retrovesical hydatid cysts, with special emphasis on differences between primary and secondary cysts.

Methods: Nine patients with retrovesical hydatid cyst were treated from 1994 to 2011 in Al-Ramadi teaching Hospital at Anbar Governorate, Iraq. Clinical, laboratory, radiological data and surgical findings were analyzed

Results: Nine patients were 5 males and 4 females with mean + SD age 39.88 + 13.99 years were included. Six patients had primary cysts (no past history of hydatid disease), presented mainly with obstructive urinary symptoms (one of them was female presented with acute retention and another with big palpable pelvic mass with right leg edema). Three patients had secondary cysts presented mainly with pelvic pain, with history of surgery on hydatid cyst before. All patients were treated by open surgery. In primary cysts: 5 patients were treated by partial cystopericystectomy and one by closed total cystopericystectomy. In secondary cysts: 2 patients were treated by open total cystopericystectomy and one by partial cystopericystectomy. There is no recurrence of the disease during mean follow up period of 19 months.

Conclusion: Retrovesical pelvic hydatid cysts have varied and non-specific presentation. Ultrasonography and computed tomography seem enough to give suggestion of these cysts in endemic area preoperatively. Primary cysts presented with more obstructive urinary symptoms earlier even with smaller sizes, than the secondary ones. Mostly secondary cysts could be treated by total cystopericystectomy, and primary cysts by partial cystopericystectomy with some exceptions like two extremes size of cysts or presence of adhesion to vital organs or from previous surgery.
