Treatment of Rhinoscleroma An Open Labeled Therapeutic Trial

Abstract

ABSTRACT

Background: Rhinoscleroma is a chronic granulomatous condition of the nose and upper airways caused by infection by the bacterium Klebsiella rhinoscleromatis. The disease requires a long course of antibiotic treatment and if not treated well it ends in stricture and stenosis of the airways which may need surgical correction.

Objectives: Comparison of two antibacterial regimens.

Methods: Two regimens of treatment were used to treat rhinoscleroma in two groups of patients with rhinoscleroma seen from 2003-2011 in Al-Kindy teaching hospital, Al-Karkh general hospital and Karbala general hospital. Diagnosis was established by biopsy, first group (8 patients): Streptomycin 1 g every other day plus tetracycline 2 g daily, second group (8 patients): ciprofloxacin 500 mg twice daily, treatment given for two months, follow up for one year

Results: Four patients (50%) of (1st group) patients were cured, the other (50%) had residual disease, and two of them developed airway obstruction which necessitates tracheostomy for one of them. Six patients of (2nd group) patients were cured by the end of the treatment regimen (75%), the other two had residual lesions needed further treatment

Conclusion: The second regimen may be better than the first one, but still the disease is difficult to treat
