Comparative Study between Early and Late Hip Spica in Treating Children (2-6) Years with Fracture Shaft Femur

ABSTRACT
Background: Fracture shaft femur in children is common and it is treated by early or late hip spica cast. Each has its advantages and disadvantages.
Objective: To compare the outcome of the two techniques.
Methods: A cross sectional study design conducted in Alkindi teaching hospital in Baghdad, Iraq from Jan 1st 2008-Dec 31st 2010. Fifty six children (2-6 years) with fracture shaft femur diagnosed by clinical examination and x-ray included in the study. Twentyeight cases treated by early hip spica and other 28 cases by late hip spica cast after a period of skin traction. The two techniques outcome measured after a period. Statistical analysis using Microsoft excel 2007, mini tab v.16 program, the statistical significant level was P = 0.05.
Results: Patients treated by early hip spica cast had no stiffness or malunion while those treated by late hip spica cast 2 cases developed stiffness, 4 cases developed rotational deformity and 2 cases had limb length discrepancy.
Conclusion: Early hip spica cast is better than late hip spica cast with much less complications.
