Modified Moufarrege Total Posterior Pedicle Reduction Mammoplasty for Gigantomastia

ABSTRACT
Background: Gigantomastia is one of the difficult problems in plastic surgery. Different
procedures had been designed for treatment. Moufarrege reduction mammoplasty is one
of them.
Objectives: To evaluate the immediate and intermediate outcome of new modification of
Moufarrege reduction mammoplasty designed for gigantomastia.
Methods: A prospective study conducted in Al-Yarmook teaching hospital and Dijlah
private hospital, from January 2012 to January 2015. Fifty patients had been included in
this study, all of them complaining from gigantomastia, seeking reduction mammoplasty. A
new modification of Moufarrege reduction mammoplasty had been done. It involved
planning, flaps dissection with the new modification of minimal or no superior glandular
resection and minimal upper skin undermining for all cases, with skin closure in almost
vertical line. Patients were followed up for about one year regarding immediate and
intermediate results and complications.
Results: Immediate postoperative aesthetic outcome was very good regarding size,
shape, symmetry, nipple areola complex viability, sensation all over the breast, almost
short vertical scar. No evaluation had been done for the effect on lactation as none of the
patients get pregnant within the period of follow up. The size was satisfactory in 90% of
cases. The shape was good in 80% of cases. Symmetry was good in 70% of cases.
Preservation of sensation was achieved in all cases all over the breast. Nipple areola
complex partial necrosis occurred only in 20% of one nipple. Length and shape of the scar
were acceptable and almost vertical.
Conclusion: This procedure gives satisfactory results for surgeon and patient regarding
better aesthetic outcome, short operative time, preservation of normal sensation all over
the breast and minimal complication rate.
