The Role of Conservative Treatment in the Management of Chronic Subdural Haematoma

Abstract

ABSTRACT

Background: Subdural haematoma (SDH) is the collection of clot in the subdural space due to hemorrhages into the dura or arachnoid, if the symptoms occur in more than 3 weeks it is regarded as chronic.

Objective: To study the role of conservative measures in the management of chronic SDH and compare it with the surgical results.

Methods: Fifty patients diagnosed as chronic SDH from Jan. 2001 till Oct. 2008 managed conservatively in the specialized surgical hospital labeled as group A were compared with another 50 patients managed surgically in the same hospital in the same time labeled as group B. The study included age, gender, symptoms and signs, CT and MRI study, the management and comparison of the results in both cases.

Results: Patients in group A and B both show complete recovery of papilledema and consciousness with significant improvement of headache and 6th nerve palsy which were the main symptoms and signs.

Conclusion: Conservative treatment of selective cases of chronic SDH can be as good as surgical interference without exposing the patient to the risk of surgery.
